Mojave 2019-001 Security Update Causing Data Loss if Interrupted – 2019-001 FileVault can’t login
UPDATE: 01/22/20 – This problem was reported as FIXED in the latest Mojave 10.14.6 Security Update 2019-002(18G2022) &Catalina 10.15.2 Update. The fixed T2 BridgeOS version is 17.16.12551. Apple did not list the fix in the 10.15.2 or 2019-002 Security Update notes but DID put them in the AppleSeed 10.15.2 Beta 2 Update Notes. I can’t post them here, but you can check the AppleSeed Patch Notes portal or you can contact Apple Support to confirm if you need further information.
Final Verdict – After further investigation, the problem was NOT the user’s fault. After more users reported what happened during the update I found that the update stalled out during the BridgeOS update phase. The black screen is only supposed to last 2-5 minutes. In reality, the update stopped and the Mac would be on the same black screen for up to 1 hour. The user had NO CHOICE but to shut down the Mac. By that time it was already too late.
Users are Unable to Unlock FileVault 2 if the Mojave 2019-001 Security Update is Interrupted.
This issue was first reported about one month ago on the MacAdmins Slack. It was reported that after some users installed the Mojave 10.14.6 Supplemental Update #3, they were unable unlock their Mac with the FV2 Password or PRK. The issue was not widely reported though so it was thought to be a fluke.
Mojave 2019-001 Security Update
All this changed when the Mojave 2019-001 Security Update was released. MacAdmins started to report the problem again.
I have a Mac that just finished installing the 2019-001 Security Update. I can’t get past the FileVault 2 screen with the password or Personal Recovery Key.

3. FileVault 2 Encrypted Machines Only. UPDATE!*
UPDATE! – We now have two separate reports of this happening when the Mac is NOT FV2 Encrypted.
If your Mac is unencrypted you should be fine.
* I have not seen any reports as of 11/09/19, that include a T2 Mac that was not encrypted.
4. Evidence? Reports of a Black Screen Followed by User Power Off
After the reports started to roll in, we started to investigate. One of the common threads is that users reported a problem with the update during the install.
Black Screen – Users reported that the Mac looked like it powered down. They would try to power it back on, interrupting the install process.
Black Screen with Apple Logo & Progress Bar Stuck – While the Update was installing, some users have reported that the update hung or stalled out. This was followed by a power down.
5. Can’t login with FV2 Password or PRK?
After the user powered down the Mac, they reported the following.
Can’t login past FileVault 2 with my Password.
Can’t boot the Mac up with the PRK.
In this situation the Mac is unable to boot up at all. The only thing that the user can do is boot to the Recovery Partition or Internet Recovery.
After booting to the Recovery Partition we tried to first mount the disk.
This did NOT work!
You can confirm the issue by typing in diskutil ap list

Note the Encrypted line. It should say LOCKED or UNLOCKED. Instead you get ERROR -69808

7. Workarounds?
Currently no known workaround is available.
We have tried multiple things.

8. Should I block this update?
After Reviewing Multiple Reports, the issue only looks to have affected a small number of users. One MacAdmin said out of 150 machines, the issue only affected 2 of them.
I tested this issue out on a 2018 MacBook Pro.

9. Will Apple fix this issue?
The black screen BridgeOS update process has been around since 2018. Something must have changed in the 2019-001 Security Update.
If you have this issue please report it to Apple ASAP.

macOS Catalina 10.15.1 Update (19B88) is now Available.
10.15.1 is Catalina’s first Update was just released and it’s now available for download. Let’s take a quick look at the Catalina 10.15.1 Update (19B88) and see what’s new.
First thing to note is the SIZE of the update! The Point Update clocks in at 4.49gb!!! That’s over 50% the size of the main installer app! (The full installer is 8.63gb).

Speaking of size, hopefully your Mac has enough space!
When you have an update that is almost 5gb in size, you also have to take in to account how much extra free space you will need. For this update you may need up to 20gb of free space to even install the update.

If you deployed the old version of the 10.15 (19A602) Installer, you most likely do not need the new (19A603) Version. As noted above, Apple did not change the patch notes and did not mention any changes to this Build Version. The Update most likely has only very minor changes.

Enhanced Gatekeeper
Gatekeeper will ensure that all new apps you install — from the App Store or the internet — have been checked for known security issues by Apple before you run them the first time and periodically thereafter. This extends the protection from the app’s source to include automated checks for what’s in the app.
Data protections
macOS Catalina checks with you before allowing an app to access your data in your Documents, Desktop, and Downloads folders; iCloud Drive; the folders of third-party cloud storage providers; removable media; and external volumes. In addition, you’re asked before an app can perform key logging or capture a still or video recording of your screen.
Activation Lock
All Mac models with the Apple T2 Security Chip now support Activation Lock — just like your iPhone or iPad. So if your Mac is ever misplaced or lost, the only person who can erase and reactivate it is you.
Dedicated system volume
macOS Catalina runs in a dedicated, read-only system volume — which means it is completely separate from all other data and helps improve the reliability of macOS.
DriverKit and user space system extensions
Previously many hardware peripherals and sophisticated features needed to run their code directly within macOS using kernel extensions, or kexts. Now these programs run separately from the operating system, just like any other app, so they can’t affect macOS if something goes wrong.

Find offline devices
Locate a missing device even if it’s not connected to Wi-Fi using crowd-sourced location. When you mark your device as missing and another Apple user’s device is nearby, it can detect your device’s Bluetooth signal and report its location to you. It’s completely anonymous and encrypted end-to-end, so everyone’s privacy is protected.
Enhanced location notifications
Location notifications include the ability to schedule notifications for different days of the week, more useful place names, and enhanced privacy controls.

Voice Control understands contextual cues, so you can seamlessly transition between text dictation and commands. For example, say “Happy Birthday. Tap send.” in Messages, and Voice Control sends “Happy Birthday” — just as you intended. You can also say “delete that”, and Voice Control knows to delete what you just typed.

Navigation commands give you quick ways to interact with macOS and apps. You can open apps, search the web, open Spotlight, and more.
Names
You can easily navigate by telling Voice Control to select the name of an accessibility label for buttons, links, and more.
Numbers
Say “show numbers” to see numbers appear next to all clickable items onscreen. Use this to quickly navigate complex or unfamiliar apps. Numbers automatically appear in menus and whenever you need to disambiguate between items with the same name. Just say a number to click it.
Grids
If you ever need to touch a part of the screen that doesn’t have a control, use Grid overlays. Saying “show grid” superimposes a grid on your screen and allows you to precisely do things, like select, zoom, drag, and more.
Hover Text
Hover Text displays high-resolution zoom of text, text fields, menu items, buttons, and more in a dedicated window. Just press the Control key when hovering over text with your cursor, and a window with zoomed text appears alongside the standard interface — helping you stay contextually aware. Text is crisply displayed in a font and color of your choice. And you can interact with buttons and type right in the zoomed window.
Zoom Display
While using a second display, you can see the same screen up close and at a distance simultaneously. You can keep one monitor zoomed in and another at a standard resolution. Or keep a personal Mac zoomed in while giving a presentation.
Simplified tab navigation
VoiceOver users will enjoy simplified keyboard navigation that requires less drilling into unique focus groups. The Tab key more simply advances through selection of elements — such as window stoplights, toolbar buttons, and scroll bars.
Punctuation in iCloud
VoiceOver users often customize the way punctuation marks are spoken. These customizations are now stored in iCloud, giving you a consistent experience across macOS, iOS, and iPadOS.

Restore from snapshot
If your third-party software is incompatible with an update you just installed, use macOS Recovery to restore from a snapshot of your computer taken right before the installation. macOS and all your apps will work just as they did before you installed the update.
Today Apple released macOS Mojave 10.14.6 Supplemental Update #3 and Security Updates 2019-005 for High Sierra 10.13 and Sierra 10.12. If Apple’s previous update release history is any guide, 2019-005 will be the final Security Update for Sierra. Once macOS 10.15 Catalina is released in October, Mojave will be security patched for two more years. High Sierra will be supported for one year and Sierra will be dropped.

10.14.6 Update Causes a Kernel Panic When Using the Built-In FaceTime Camera.
NEW UPDATE: 4/08/20 – I am getting multiple reports that users are having issues applications that use Hardware Accelerated Video with the latest 10.14 and 10.13 2020-002 Security updates. The system will 100% freeze up during video conference, video or heavy app usage and you will have to hard power down. FINAL UPDATE: 1/22/20 – Multiple users have contacted me to let me know that the latest macOS Mojave 10.14.6 Security Update 2019-002 (18G2022) has fixed the issue for them! The T2 BridgeOS version is 17.16.12551 or newer.
UPDATE: 10/03/19 – I am hearing from readers and commenters that Zoom has confirmed that this issue is fixed in macOS Catalina 10.15. This could very well mean that we will not get a fix for Mojave. 🙁
After Apple released the 10.14.6 Update, some users of 2019 & 2018 15″ MacBook Pros started to report Kernel Panics when using the Built-In FaceTime Camera.
I reported on a similar issue about a month ago when the 2019-004 Security Update for High Sierra and Sierra started causing a Kernel Panic after waking from sleep.
In this article, I will give you the lowdown of the issue. In the end, I will also give you a workaround until Apple releases a fix.

3. This issue does NOT affect most 10.14.6 users.
I tried time and time again to reproduce this issue. Having both a 2018 and 2019 15″MacBook Pro for testing, I figured that I would be able to reproduce the issue. Having FaceTime.app open for hours and using Zoom Conference call for 4 hours, produced zero crashes. This could be a clue as to an additional cause on the reported users computers, but I’m not sure.
4. Kernel Panic Reports
What do the Kernel Panic Reports look like? Looking over the report it looks like BridgeOS is crashing. Below is a sample report.

5. Why rolling back will NOT work.
You might think, what if I roll back to a previous version of Mojave like 10.14.5? In the past, this might have worked! It will NOT work when the issue is caused by BridgeOS. Apple has made it impossible to roll back the version of your T2 BridgeOS.
After you updated to 10.14.6, your BridgeOS was also updated. If you try to reinstall 10.14.5, this version will still work but the newer BridgeOS will still be installed. This is also why if you got your MacBook Pro replaced at the Apple Store, the issue could STILL be happening to you.
The only way you could get this to work is if you got ahold of a 10.14.5 system on BridgeOS version 16.16.5601 or lower.
6. Built-In FaceTime Camera
The crash ONLY happens when you use the MacBook Pro Built-In FaceTime Camera. It does not matter what you do. From the many reports that I have looked through, the crash could happen the very second you activate the camera or 2 hours after. Most of the reports seem to be from 10min to 1 hour of use.

You get the idea, basically ANY application that uses the Built-In FaceTime Camera.
8. Workarounds
Most issues like this have some type of workaround. Sometimes a workaround is found by accident or after hours of testing. This time around a few users on the Apple Discussion Forums found at least one workaround.
Plug in an additional Web Camera. This could be a USB Web Camera or even a monitor with a built-in camera like the Apple 27″ ThunderBolt Display.
You don’t even have to use the secondary camera. For some reason with the second camera plugged in reports say that you can use the built-in FaceTime Camera without crashes. Found by user edgonz305
Run a Windows Fusion VM with Skype in that VM. The problem does not occur using integrated camera. This is not a solution for everyone, but for me it works better than using an external camera. Additional workaround from OaSOakover
You can prevent the crashes by just disabling video on the meeting. (This one is pretty obvious if you don’t need to use the FaceTime Cam)

9. Will Apple fix this issue? 10.14.6 Supplemental Update #3 ?
With the end of support coming up fast for Mojave, will Apple fix this issue? I think the answer is YES. This is coming from reports that Apple DOES know about the issue and is still investigating. My guess is that Apple will release one final Supplemental Update to fix this.

Apple Today Released macOS Mojave 10.14.6 Supplemental Update #2 (18G95)
Apple has now released two Supplemental updates for macOS Mojave 10.14.6. The first macOS Mojave 10.14.6 Supplemental Update #1 (18G87) was released on August 1st. You can take a look at what was included inside the first Supplemental Update in the link below. The second one, which was released today is called macOS Mojave 10.14.6 Supplemental Update #2 (18G95). Apple does not call the updates #1 or #2 but I am so you can understand that two different Build Versions are out there.
The macOS Mojave 10.14.6 update improves the stability and reliability of your Mac, and is recommended for all users. This update:
Resolves an issue that may cause certain Mac notebooks to shut down during sleep
Fixes an issue that may degrade performance when working with very large files

When the 2019-004 Security Update wake from sleep Kernel Panics started to happen, most of the reports were from 10.13 High Sierra. I did see a few from 10.12 Sierra and a few from 10.14 Mojave. The issue was coming mostly from users reports on Apple’s Discussion Forum, Reddit, MacRumors & Emails / Comments to me.
Is this issue the same as the 2019-004 Sleep/Wake issue?
I’m not sure if Mojave had the same issue, but it sure looks like it. I tried many times to reproduce this issue but couldn’t on multiple Mac models. Most likely, this means that a very particular software setup caused the problem.

My guess was once the Mac reached out to software update it would find the new version of 10.14.6 and download it. Pcrandom ran this test and found that this is, in fact, true with one catch. The old version was still in /Library/Updates.

I wanted to test installing the new update to see if the old one was removed, but we did not get a chance to test.
Keep in mind that in 10.14 /Library/Updates is SIP protected, so you will not be able to delete the old update if it remains!
Softwareupdate knows all.
If you are using softwareupdate to deploy updates to your Macs, you will be fine. It will let you know which update you need to install. If you want to check just run a quick softwareupdate -l and you will see what’s available for your particular situation.

Apple today pulled the 2019-004 Security Update after it was found to cause Kernel Panics when a Mac wakes from sleep.
First spotted on Apple Support Forums by user Fugge on July 10th, 2019. He installed the 2019-004 Security Update Developer Beta and started seeing Kernel Panics crashes after waking from sleep. A Kernel Panic will happen every single time the Mac wakes from sleep. The issue was then reported on July 24th by Howard Oakley

Apple today at 12:05 CST re released the 2019-004 Security Update for High Sierra 10.13 and Sierra 10.12. A new BridgeOS and EFICheck AllowListAll update are also new. I will update when I have new build versions and new information.
The updated version of 2019-004 will now show up new for you in software update. Even if you installed the 2019-004 update before it was pulled, you will see the update show as NEW!
I already have one report from a user saying that the new update has cleared up the crash on sleep issue!
BridgeOS was re released but the BuildVersion remains the same at 16.16.6568 the updated files inside are dated 7/26

Apple has pulled the 2019-004 High Sierra update from the Software Update Service. It will not show up if you run softwareupdate -l or show up at the old address support.apple.com/kb/DL2012?viewlocale=en_US&locale=en_US. The download button still works when you go to Apple.com/downloads.

4. 2019-004 Sierra Update Removed from SUS.
Apple on late on Wednesday also removed the 2019-004 Sierra 10.12 Security Update. This update was removed hours after the 10.13 update was removed. The same problem must be in this update for Sierra Systems. Keep in mind only the 2016 and 2017 MacBook Pros can install Sierra 10.12.
5. BridgeOS version 16.16.6568 also removed.
If you downloaded the 2019-004 update before Apple removed it, you still would not be able to install it if you have a T2 equipped Mac. BridgeOS version 16.16.6568 with a Product ID of 041-51542 is now deprecated. When the Mac starts the installer the new BridgeOS is REQUIRED before the security update can install.
This also means you can not install the 10.13 or 10.12 2019-004 Security Update on T1/T2 Macs.
6. Check your DEP or Provision work flow. Software Update for 10.12 and 10.13 will not show previous Security Updates.
If you have a provisioning workflow where you rebuild Macs using DEP or some other type of system, you will be stuck on the current BuildVersion on the full installer BuildVersion. This means if you run a softwareupdate -iar command or have automatic updates set on, it will not receive any security updates until a fixed 2019-004 update is released.
7. Affected Mac Models
I am not totally sure the scope of this, as users have reported the issue mostly on MacBook Pro’s. Others reported this issue happening on iMacs.
I have confirmed the issue does not happen on at least one Non T1/T2 Mac. I was able to install 2019-004 on a 2017 MacBook Air and it did NOT Kernel Panic on sleep or wake.

2019-004 kernel panic crashes
9. Fresh install of 10.13 + 2019-004 Security update does NOT crash on sleep!
I was able to update a 2018 T2 MacBook Pro with the 2019-004 update and BridgeOS 16.16.6568 and it does NOT crash on sleep wake. The issue is starting to look like a conflict with a Kernel Extension or piece of 3rd party software. A few of the Kernel Panic Reports were sort of pointing to Virtual box but that may be pushing it.
10. Workarounds? “Prevent computer from sleeping when display is off”& Resetting SMC
1. Try turning on “Prevent computer from sleeping when display is off” NOTE: This option is only available for when you are plugged into power. I am not sure what will happen when you are on battery power.
I was able to get the shutdowns to stop by disabling computer sleep when display is off. system preferences -> energy saver -> check “Prevent computer from sleeping when display is off”
Not a solution, but at least stops the annoying shutdowns until Apple can fix the issue.
2.Reset SMC (System Management Controller)NOTE: this only seems to help about 50% of the time. Resetting the SMC seems to work for some but not others.
I found a workaround that actually did the trick. SMC Reset, which for me was as simple as shutting down the device, unplugging the power, waiting 15 seconds, and plugging it back in. Directions called for me to wait 5 seconds before turning the power back on, but in my case it booted right up automatically after reconnecting the power cord. 3. Use Apple Menu > Sleep, instead of closing the lid.
I can put the display to sleep and leave the lid open without a problem. I recommend doing this if you are connected to power.

4. Wait until Apple Fixes the issue and then releases a new Updated Security Update.
5. Fresh reinstall of 10.13.6 and 2019-004 Security Update. NOTE: read section 8.

7. UPDATE 7/25: THIS DOES NOT FIX THE ISSUE!!!While the restore does work the system will still crash. Check to see if you tmutil created a time machine restore point in the Recovery Partition by running tmutil listlocalsnapshots /
You should see something like this

If you have a snapshot listed you can boot to the Recovery Partition, click on Time Machine and if you have a localsnapshot listed it will look like this.
You may be able to revert back to a the previous OS Version if you can get to the localsnapshot before 24 hours.
If you have successfully reverted back you can click about this Mac to verify you are on the old build version. You can also type in sw_vers in terminal to display the BuildVersion.

Now that we made it back to 17G7024 the question remains… Is the actual issue in the 2019-004 Security Update OR in the T1 EmbeddedOS or T2 BridgeOS? UPDATE: looks the the issue still remains. Continue reading below.
11. Reports are saying that the problem remains after reverting back to (17G7024)
A user in the Apple Discussion Forum was able to follow the above instructions and use the TM localsnapshot to revert back, but reported that he is still having the issue.
So, I reverted to the snapshot that was taken right before the update, but the problem still exists. I never saw this issue before installing the security update.
12. Is the problem actually in the T1/T2 Chip update?
This could very well mean that the problem is not inside the 2019-004 update code but in the T1/T2 chip update known as EmbeddedOS or BridgeOS. This would make sense since you can still install the 2019-004 update on a NON T1/T2 system and it’s fine. You can’t downgrade to a previous version of the T1/T2 Update. If this is the case you will have to wait until Apple releases the fixed 2019-004 update which includes a updated T1/T2 Update.
13. This has happened before. It’s recommended that you wait a few days before you install any update.
This is not the first time Apple has pulled an update. They pulled High Sierra and Sierra 2019-002 Security Update this past March. It’s a good idea to wait at least a few days before you update.
Today Apple released macOS Mojave 10.14.6 and Security Updates 2019-004 for High Sierra 10.13 and Sierra 10.12. If Apple’s previous update release history is any guide, 10.14.6 will be the final update for Mojave. Once macOS 10.15 Catalina is released in September, Mojave will be security patched for two more years. High Sierra will be supported for one year and Sierra will be dropped.
